Guthrie East Corning OB/GYN is seeking a compassionate Registered Nurse to join our MOMS Program as an RN Care Coordinator. In this unique role, you will serve as an advocate, educator, and care navigator for pregnant women receiving prenatal services, helping ensure they have access to the support, resources, and clinical care needed for healthy pregnancies and positive outcomes.

This position combines nursing, case management, patient education, and community outreach, allowing you to build meaningful relationships with patients throughout their prenatal and postpartum journey.

Position Summary

Acts as an advocate to low income women receiving prenatal services through the MOMS Program. Completes assessment and makes appropriate referrals when needed.

Education, License & Cert

Must be a Registered Nurse in the state of practice. Bachelors' Degree of Nursing Required.

Experience

Experience preferred but not required.

Essential Functions

Assesses follow through on clinical recommendations to client for the health and well‐being of the client and the fetus. Submits quarterly HIV counseling and testing results to NYS. Assesses the client’s strengths and coping strategies both immediate and through extended family supports and creativity in utilization of limited material resources. Initiates a plan of care in conjunction with the client and OB provider that is agreeable and realistic. Performs initial intake assessment and ongoing assessment with client to identify nutritional, financial, psychosocial and obstetrical risks/problems utilizing the Care Plan and Risk Assessment. Provides positive health education during client appointments including but not exclusive to topics listed in the Care Plan. Provides referrals to support services (ie. Social worker, nutritionist) and other community based resources and maintains follow‐up on referrals. Provides follow‐up on missed appointments. Provides monthly summary of program status to administrative supervisor and Chief of OB/GYN department. Provides primary nursing through case management.

Other Duties

Administers prescribed injections/medications and records them in patients chart. Provides continued educational services and support for the client during post‐partum period. Arranges for services not provided by the qualified MOMS provider including alcohol/drug treatment, abuse, mental health or dental services. Arranges for transportation to prenatal appointments and to the hospital in an emergency. Assures the client receives personalized, coordinated, and comprehensive services to provide her with a secure knowledge base to make decisions regarding her well‐being. Attends state training programs for MOMS Coordinators. Conducts Fetal Non Stress tests as needed. Enrolls the Guthrie system providers eligible to be MOMS providers. Initiates all billing of prenatal services received through the Guthrie Clinic MOMS program. Initiates outreach to community on MOMS program and access to the program.

The pay range for this position is $68,120.00 - $91,291.20

Requirements

Must be a Registered Nurse licensed in the state of practice with a required Bachelor's Degree in Nursing. While experience is preferred, it is not required for this position.
