About the role

Provides direct and indirect nursing care for pediatric enteral patients, including clinical case management and the setup of enteral pumps and tubes. Coordinates with physicians and dieticians to ensure optimal nutritional support and manages the education of caregivers.

Schedule: Monday-Friday, 8:00am to 5:00pm

A Brief Overview Provides direct and indirect nursing care in a professional manner based on the Physician Plan of Treatment and the Nursing Care Plan.

Essential Functions

Provides direct and indirect nursing care in a professional manner based on the Physician Plan of Treatment and the Nursing Nutritional Care Plan.

Provides clinical case management of homecare enteral patients to include, but not limited to:

Assures authorization of all enteral therapy with all required payor sources.

Obtains new enteral therapy referrals and arranges training and set-up in conjunction with home health nursing department.

Performs enteral set-ups on patients to include education/training on pumps, NG tube insertion, syringe feeding, etc. depending on physician’s order, in coordination with HH Nursing.

Reviews and assists in completing cover letters for physician enteral orders to be mailed to physicians.

Assists in updating enteral notes in CPR+ based on physician orders/ prescriptions.

Evaluates usage of patient enteral supplies and equipment to ensure appropriate supplies are available and/or to identify issues requiring updates to treatment plan.

Ensures proper enteral kits/supplies are provided for patients.

Participates in monthly billing of enteral per diem charges to ensure accuracy.

Assists in the monitoring of patient response to enteral therapy provided by CHHC.

Communicates with physician, dietician, and other healthcare providers to ensure optimal nutritional support for patients.

Maintains ongoing communications with HH-Pharmacy Manager concerning enteral patient status and any information which may influence or change provision of services.

Provides support to enteral team and Home Health Nursing Department in education of enteral products, pumps, and therapies.

Ensures appropriate patient enteral teaching materials are available by developing/updating materials or obtaining from vendors.

Completes discharge summary and instructions to caregivers when patient is discontinued from enteral pump.

Conducts an individualized age-specific accurate assessment of developmental, cognitive, cultural, psychosocial, spiritual, family influences and physical/medical needs.

Demonstrates clinical competence in administering cares according to physician orders, nursing care plan and policies.

Performs and carries out cares and treatments according to company policies and procedures.

Demonstrates knowledge of appropriate care for infant, toddler, preschool, school age and adolescent patients.

Implements nursing actions in accordance with the plan of care and with respect for child/family individualized needs by incorporating child’s and/or caregivers preferences into care.

Assesses patient/caregivers for understanding of treatment regimen and ability to perform cares. Provides ongoing education to patient and caregivers using an individualized approach in delivery style.

Monitors and documents response to pain, cares, treatments, and medications and evaluates outcomes of care provided, adjusting care process as indicated.

Performs reassessments based on PPOT, company policy and changes in child’s condition. Communicates changes to appropriate people (MD, family, Primary Nurse, office) as indicated.

Demonstrates age-specific competency in assessments of patients by the ability to obtain and interpret information in terms of patient needs, knowledge, growth and development, and in understanding the range of treatment needed by the patient.

Knows emergency preparedness plan and responds appropriately in crises situations and/or life-threatening emergencies.

Documentation reflects accurate assessment/evaluation, modification, patient’s response to therapy, changes in conditions, and therapeutic interventions/procedures performed.

Entries in medical record follow company policy such as correct date, time, and use of appropriate company approved abbreviations, legibility, etc.

Documentation is completed within established timeframes including, but not limited to : Initial Assessment, Nursing Flow Sheet, Progress Report Sheet, Teaching Log, Medication Record, Medication Profile, Developmental Assessment, Incident Report Forms, etc.

All charting is completed per policy to reflect accurate care on proper forms, according to internal and external regulatory requirements and MD orders.

Completes and verifies accurate MD orders entry into computerized orders and PPOT per policy.

Reviews and makes appropriate changes in the computerized nursing care plan and time schedule when patient’s condition/routine changes to accurately reflect current plan of care.

Obtains all enteral referrals, initiates new paperwork and ensure paperwork is complete and accurate.

Develops, implements and evaluates individualized patient/family nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs.

Utilizes assessment data obtained to individualize appropriate interventions and goals for the patient, in collaboration with the family and other members of the interdisciplinary healthcare team.

Formulates goals and outcomes based on the assessment data per policy.

Maintains and oversees that the nursing and interdisciplinary plan of care/goals are coordinated, implemented, evaluated and revised based on patient’s needs and per policy.

Provides and/or coordinates, patient/family education with other disciplines, according to identified care needs taking into consideration assessment of patient/family’s readiness and their ability to learn, per policy.

Provides direct and indirect work supervision of the Enteral LPN.

Provides training.

Serves as the resource expert for questions and advice.

Provides input to the Manager of HH Pharmacy on job competency.

Regular attendance at work is an essential function of the job.

Perform physical requirements as described in the Physical Requirements section

Checks/reads e-mails daily.

Accepts and completes other assigned tasks in a professional manner.

  • Associate's Degree from an accredited school of nursing Required or
  • Bachelor's Degree from an accredited school of nursing Required
  • Experience Qualifications
  • Minimum two years nursing experience Required and
  • Experience with pediatric patients Preferred and
  • Experience in homecare environment Preferred

Skills and Abilities

Mathematical skills necessary for addition, subtraction, multiplication, and division.

Basic skill level in computer software including Microsoft Outlook, Word, Excel, etc.

Ability to communicate successfully both orally and in writing, including effective listening skills.

Knowledge of clinical documentation protocols and home health care regulations and standards preferred.

Current clinical knowledge regarding specific treatment modalities, therapeutic applications, OSHA guidelines and drug therapy parameters.

Ability to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure.

Remains calm and professional in stressful situations

Excellent interpersonal skills and ability to work effectively with others.

Ability to function independently in a home setting.

  • Licenses and Certifications
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure valid in the state of Nebraska Required and
  • BLS - Basic Life Support through the American Heart Association Required and
  • PALS - Pediatric Advanced Life Support Preferred
  • A valid driver’s license and an acceptable motor vehicle record/driving record Required and
  • Must provide own vehicle to travel between home health patients Required and
  • Associate/diploma nurses not eligible for transfer Required

Requirements

Requires an Associate's or Bachelor's degree in nursing, a valid RN license in Nebraska, and BLS certification. A minimum of two years of nursing experience is required, with pediatric and homecare experience preferred.
