Provide skilled physical therapy services through home visits within designated territories. Perform comprehensive evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities. Develop individualized treatment plans based on patient needs and physician orders. Implement evidence-based therapeutic interventions, including therapeutic exercise, gait training, balance activities, and mobility training. Provide integumentary and wound prevention interventions as clinically appropriate. Collaborate with nursing and interdisciplinary team members to coordinate patient care. Educate patients, caregivers, and family members to promote safety, independence, and successful recovery at home. Continuously assess patient progress and modify treatment plans as needed. Accurately document evaluations, interventions, and patient progress using electronic medical records and tablet technology.
Current New Jersey Physical Therapist (PT) license. Minimum of one year of recent clinical experience as a licensed Physical Therapist. Graduate of an accredited Physical Therapy program. Ability to work independently and effectively manage time. Strong communication, organizational, and problem-solving skills. Electronic medical record (EMR) experience preferred.
