The Referral Admissions Coordinator responds to new referrals to identify patient needs and admit eligible patients into hospice care. They coordinate services and educate patients, families, and referral sources about hospice benefits and options.

The Referral Admissions Coordinator is responsible for responding to new referrals to identify needs, offer hospice solutions, and admit medically eligible patients into Hosparus Health care.
The Referral Admissions Coordinator coordinates care and services, educates referral sources, patients, and their families about the hospice benefit and options available.
The Referral Admissions Coordinator strives for the goal of providing premier customer service.
Referral Admissions Coordinator Position Qualifications: RN with current RN licensure in the states of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license upon hire.
1 year of nursing experience; ER, ICU, Home Health experience a plus.
High level of computer skills to navigate multiple screens to complete admission.
Proven time management skills and ability to toggle between multiple tasks.
Ability to work autonomously and recognize when other disciplines are needed.
Must have a valid driver’s license.
Knowledge of Medicare/Medicaid reimbursement helpful.
Knowledge of palliative care helpful.
Use of technology-based tools, including electronic medical record, strongly preferred.
