About the role

As a Virtual Float Advanced Practice Clinician (APC), you are a flexible capacity anchor and coverage partner for our longitudinal care pods. You do not carry a permanent, dedicated patient panel. Instead, you step into other providers' panels to deliver high-quality virtual care during their absences, working frequently alongside an in-home Health Advocate.

To ensure clinical continuity and smooth operational handoffs, each part-time clinician in this pool allocates a designated, set number of hours per week specifically to inbox management, urgent clinical triage, and results follow-up for the covered panels.

How Coverage is Scheduled

  • Coverage operates on a predictable, month-ahead framework to respect your schedule:
  • Planned
  • Coverage: Your clinical blocks are confirmed at the start of each month to backfill known PTO and planned absences
  • This forms the basis of your committed hours
  • Dedicated
  • Inbox
  • Windows: A fixed portion of your weekly hours is pre-scheduled to manage the floating inbox queue, ensuring care loop closure
  • Short-Notice
  • Flex (optional): You may opt into pre-agreed day-time flex windows to be activated for same-day call-outs
  • This is voluntary, defined in advance, and operates strictly within core weekday business hours

Responsibilities

will include:

  • Essential Functions
  • Patient Care & Panel Coverage
  • Conduct comprehensive virtual onboarding, annual, transition-of-care, and follow-up visits - facilitated by an in-home Health Advocate or fully virtual - completing biopsychosocial assessments and managing complex chronic conditions (hypertension, diabetes, COPD, heart disease)
  • Order and follow up on labs and imaging to inform immediate treatment plans
  • Deliver targeted health coaching, lifestyle counseling, and disease-specific education to support patient self-management
  • Execute thorough, documented clinical handoffs back to the primary longitudinal care pod at the close of each coverage block
  • Systematic Inbox & Triage Management
  • Dedicate a set number of scheduled hours each week to clearing the centralized Float Pool queue: signing off normal results, managing critical medication refills, contacting patients with updates on abnormal results , and routing complex issues to permanent care teams
  • Provide rapid results management and urgent clinical triage for virtual and hybrid providers during their absences
  • Quality & Collaboration
  • Support preventative-care quality campaigns, including focused outreach and HEDIS/STARs gap closure
  • Provide real-time clinical guidance to Health Advocates during virtual visits; coordinate with behavioral health, nursing, pharmacy, and external providers
  • Possible attendance of weekly Multidisciplinary Care Conferences (MDCC) as needed to support care coordination for complex patients
  • Maintain accurate, timely, compliant EMR documentation with rigorous diagnostic coding and adherence to evidence-based care plans

Desired skills and experience

  • Required
  • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license
  • Active
  • NP/PA licensure in at least one of the following core markets: MI, MA, or CO (with a willingness to obtain additional state licenses as requested)
  • Active DEA registration and state controlled-substance license.
  • Current BLS certification
  • 3+ years of experience in adult primary care, family medicine, urgent care, or emergency medicine
  • Preferred
  • Prior high-volume virtual/telehealth care experience
  • Working knowledge of value-based care, HEDIS/STARs, and clinical documentation improvement (CDI)
  • Hands-on experience using AI tools in a clinical setting to optimize efficiency

Requirements
