Collaborates with a multidisciplinary care team to coordinate outpatient services and ensure appropriate levels of care for patients. Responsibilities include gathering clinical data, utilizing patient registries, and providing direct crisis intervention and de-escalation.
The Assistant Care Manager reports to the Healthy Young Minds Clinical Manager. The Assistant Care Manager has frequent contact with patients, families, physicians, the interdisciplinary team, schools, ancillary services, and community resources.
In addition to meeting with patients and families, this position is responsible for gathering and reporting patient functioning data and clinical measurements, utilizing the patient care registry, communicating between collaborative care team members, direct patient contact to assigned patients and family members, and direct intervention with patients and families, to include crisis intervention and de-escalation where required.
