Concerto is seeking a PRN Renal Dietitian to join its team! The PRN Renal Dietitian is responsible for providing nutrition education, counseling and interventions as appropriate to End Stage Renal Disease (ESRD) patients and authorized caregivers in clinical settings to maximize the patient’s nutritional well-being. What Will You Do? 1. Nutrition Assessment & Care Planning Conducts initial and ongoing nutrition assessments, develops individualized care plans, and updates them in collaboration with the interdisciplinary team (IDT). 2. Medical Nutrition Therapy & Monitoring Designs and adjusts nutrition interventions based on lab results, dialysis adequacy, and patient goals. Monitors progress and documents monthly. 3. Patient Education & Counseling Provides tailored nutrition education using appropriate materials, considering each patient’s medical, social, and economic background. 4. Team Collaboration & Facility Support Participates in care conferences and quality improvement meetings. Travels to assigned facilities and provides coverage as needed. 5. Professional Development & Compliance Maintains credentials, stays current with clinical practices, and adheres to CMS regulations and company standards.QualificationsYou Might Be a Good Fit If… Compassionate and Empathetic A Critical Thinker and Problem-Solver An Individual who Thrives in a Team Environment An Individual with Excellent Communication Skills Purpose-driven What’s Required? Bachelor’s degree in Nutrition, Dietetics, or similar area required. Registered Dietitian with at least 12 months’ clinical experience as required by CMS. State licensure or certification required if licensure or certification is available in the state where the facility is located. Intermediate computer skills and proficiency with MS Word, Excel, Outlook required as proficiency with Concerto specific applications within 60 days of employment. Requirements

Candidates must hold a Bachelor's degree in Nutrition or Dietetics and be a Registered Dietitian with at least 12 months of clinical experience. State licensure or certification is required where applicable, along with proficiency in Microsoft Office.
