This is a PRN/Remote opportunity for approximately 0-40 hours/week. Key Responsibilities Conduct nutrition assessments and documentation Complete MDS assessments and care plans Monitor weights and nutrition risk Participate in nutrition at-risk meetings Strong communication with the IDT
