Description Title: Counselor
Collaborate with client and family to identify goals and potential solutions to problems which cause emotional turmoil. Counselors seek to; improve communication and coping skills; strengthen self-esteem; promote behavior change; and provide accurate documentation of efforts.

Job functions, responsibilities and authority: Working from a client’s Individualized Treatment Plan, counsel individuals, couples, and families as assigned using evidence-based treatment techniques. Complete Comprehensive Diagnoses for clients. Complete required assessments for qualifying clients. Along with client and/or parent/guardian, determine objectives and tasks for clients; develop Psychotherapy treatment plan. All other duties as assigned. Attend all required meetings and trainings. Document and submit all clinical progress notes by the following Sunday night at 11:59 PM. Review counseling clients’ psychotherapy plan every 90, 180, & 270 days. Perform annual renewals for assigned clients and submit applicable documentation within the appropriate timeframe. Comply with any Insurance Carrier requirements to treat. Communicate pertinent treatment information to client, family, multidisciplinary team, and supervisor(s). Report abuse as needed (see Tueller Counseling Services Policy and Procedure #320) Maintain all required documentation for employee file including a current enhanced background check and licensure.
