The Occupational Therapist performs comprehensive initial and ongoing assessments for patients in their home settings. They collaborate with interdisciplinary teams to develop and implement physician-ordered plans of care.

Current license in the State of Minnesota as an Occupational Therapist (OT).
Two (2) years of experience as an Occupational Therapist.
Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance required.
CPR Certification
Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
Participates in ongoing interdisciplinary assessment of the patient.
Provides services that are ordered by the physician as indicated in the plan of care.
Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the service area of our patients is the 11 counties of the Twin Cities Metro Area.
