Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
You are a motivated Speech Therapist who shares our vision of delivering home care. You treat patients in a caring and compassionate manner and help them achieve their goals of staying in their own home whilst receiving treatment.
This role comprises 100% travel within our service region. You will be traveling to patients’ homes.
We have several counties that we service across Northern California specifically Butte County, Shasta County and Yuba Sutter counties and surrounding areas such as Colusa, Glenn and Tehama.
This is a Part-time role, with the possibility of expanded hours in the future.
To perform this job successfully, you must be able to perform the following duties successfully: Conduct in-home assessments and speech-language evaluations Develop and implement individualized treatment plans Provide therapy focused on communication, cognition, voice, and swallowing disorders Educate patients and caregivers on exercises and communication strategies Maintain clear and timely documentation in accordance with home health regulations Collaborate with nurses, physical/occupational therapists, and physicians as part of an interdisciplinary team.
Master’s degree in Speech-Language Pathology from an accredited program Active California SLP license Current CPR certification Minimum of 1 years’ experience, preferably in home health or medical setting Valid driver’s license, reliable transportation, and willingness to travel Eligible to be insured under our liability policy with a clean DMV report Excellent communication, interpersonal and time management skills Ability to work independently and as part of a multidisciplinary team
Conditional job offer is successfully passing a medical exam. Physical requirement for this role specifically includes Sits, stands, bends, lifts, walks, and moves intermittently during working hours. Able to lift 50 pounds without assistance. Able to drive intermittently daily
Pay range is $55.00 to $65.00 per hour. Exact compensation may vary based on skills, experience, and location.
