Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for Home Health Aides to join our award-winning team.

Key Responsibilities Adhere to care plan to ensure safe and competent care for the patient.

Assist patients with personal hygiene and support the maintenance of a clean, safe, and healthy environment.

Help patients with ambulation as prescribed by the physician and approved or supervised by a clinician or therapist.

Support therapy personnel as needed during rehabilitative processes.

Encourage patients to achieve maximum independence according to the nursing care plan.

Promote mental alertness by engaging patients in activities of interest.

Provide emotional and psychological support to patients and household members, fostering trust and confidentiality.

Report any changes in the patient’s condition or home environment to the RN or clinical manager.

Perform light housekeeping tasks related to creating a safe and comfortable environment, as directed by the clinician.

Complete visit reports promptly and accurately within the electronic medical record system.

Confirm daily visit schedules to ensure seamless coordination with other staff.

Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.

Understand and adhere to the agency’s process for managing and addressing customer complaints.

Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.

Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.

Requirements

Qualifications

Must have a CNA or HHA license. Must have completed a minimum of 75 hours of training, which includes an introduction to personal care services. Must be free from health problems that may be injurious to patient, self, and coworkers and must present appropriate evidence to substantiate this. Ability to comprehend the basics of personal care, housekeeping, and meal preparation. Must understand and respect patient needs including ethics and confidentiality of care. Must have a valid driver’s license and access to a reliable and insured vehicle. Ability to visit homes that may not be wheelchair accessible. Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques. Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
