About the role

The technician is responsible for performing medication reconciliation for high-risk patients by obtaining comprehensive medication histories. They document these lists in the electronic health record and communicate discrepancies to the patient's care team.

About Us

The Inpatient Pharmacy Department at Valley Children’s utilizes evidence-based therapeutic guidelines to provide exceptional pharmaceutical care to our patients. Our highly trained pharmacists and pharmacy technicians work side-by-side with providers and other interprofessional teams to ensure optimal patient outcomes, while utilizing state-of-the-art technology to maximize patient safety. Inpatient Pharmacy provides clinical interdisciplinary pharmacy services in all inpatient areas including Rehab, PICU, NICU, Acute Care, and Oncology. We have numerous pharmacy advance practice protocols including antibiotic monitoring, anticoagulation, IV to oral conversion, and renal dosing. Other clinically supported services include the Emergency Department, Palliative Care, Investigational Drug oversight, and parenteral nutrition (including production).

Our team members also support the following specialty clinics: Oncology, Hematology, Rheumatology, Pulmonology/CF, Endocrinology, Neurology, Cardiology and our complex primary care center. Many pharmacists hold collaborative practice agreements to optimize medication therapy management and collaborate to enhance patient care.

Job

Summary: This position is accountable for participating in medication reconciliation for patients who have been identified as high risk for medication discrepancies. This role involves obtaining comprehensice medication histories interviews with patients caregivers, as well as communication with outpatient pharmacies and other sources. The technician accurately documents and updates the medication list in the electronic health record. The technician will communicate completion of medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist). The technician will also be responsible for participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.

Qualifications

Education High School Diploma/G.E.D. (required)

Licenses and Certifications CPHT - Certified Pharmacy Tech - PTCB - Pharmacy Technician Certification Board (required) TCH-Pharmacy Technician License - PTCB - Pharmacy Technician Certification Board (required)

Work Experience Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience. (required)

Skills and Abilities Computer Skills Proficiency with the Microsoft Office Suite. Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses.

Requirements

Candidates must have a High School Diploma and be a Certified Pharmacy Technician (CPHT) via the PTCB. A minimum of three years of experience in hospital, clinic, or retail pharmacy is required.
