About the role

Provide comprehensive primary care to a diverse adult population, managing chronic and acute conditions. Partner with the care team to drive quality outcomes under a value-based care model using eClinicalWorks.

We are looking for a driven, compassionate Nurse Practitioner or Physician Assistant to join our Primary Care team and help us deliver the kind of care patients remember and recommend.

  • AM to 5:00 PM. No nights, no weekends. Practice at the top of your license. You will manage your own panel and make the calls, with collaborative physician support when you want a second set of eyes. Be part of the shift to value-based care. Your bilingual skills will make a real difference every single day. What you'll do Provide comprehensive primary care to a diverse adult patient population Manage chronic and acute conditions, preventive care, and wellness visits Partner with our care team to close gaps in care and drive quality outcomes under our value-based care model Document efficiently and accurately in eClinicalWorks (eCW) What you bring Active Florida license as a Nurse Practitioner (ARNP/APRN) or Physician Assistant (PA-C) Minimum 3 years of experience in Primary Care or Internal Medicine Familiarity with the value-based care (VBC) model Bilingual (English and Spanish) Hands-on experience with eClinicalWorks (eCW) The confidence and clinical judgment to function independently

Requirements

Must have an active Florida license as an ARNP/APRN or PA-C with at least 3 years of experience in Primary Care or Internal Medicine. Proficiency in Spanish and experience with eClinicalWorks are required.
