About the role

This role focuses on promoting patient and family wellness by developing comprehensive transitional care plans for patients with complex psychosocial needs. The position involves collaborating with physicians, external agencies, and service team members in Inpatient and Emergency Department environments.

Overview This position strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments. Works to promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.

Responsibilities

Qualifications

Education Master’s Degree in Social Work from an accredited social work program Required Work Experience 1 year of experience in an acute or post-acute setting Required Licenses and Certifications None Required LMSW - Licensed Masters Social Work Preferred Accredited Case Manager (ACM) Preferred CCM - Certified Case Manager Preferred Business
