Comp: $82.50 / point We're fixing home healthcare, and we're building our presence in Detroit Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. Spend more time caring for patients—not charting. At Adaptive Home Health, we're excited to offer our clinicians an innovative AI-native home healthcare platform designed to simplify your day. Enjoy customized schedules, optimized routing for greater efficiency, and ambient AI scribing that automatically documents your visits while you focus on your patients. Prefer not to record during a patient visit? No problem. Our platform also includes an after-visit voice documentation option, allowing you to capture your notes afterward and leverage AI to draft your charts. By streamlining documentation and reducing after-hours charting, our technology helps you spend less time on paperwork and more time doing what you do best, providing exceptional patient care. The mission of this role Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

Carry a high-quality caseload: Minimum of 10 productivity points/week at target visit quality.

Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

Own outcomes, not just visits: you are accountable for the patient’s SLP plan, progression, and functional outcomes. Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits.

Assess and treat (as ordered/clinically indicated): Aphasia and other communication disorders Cognitive-communication impairment (attention, memory, executive function) impacting safety and independence Dysphagia screening/assessment and swallow safety education (within home health scope and orders) Build and execute a clear plan of care with measurable, functional goals.

Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. Coach patients/caregivers with practical strategies and routines that actually get done. Document promptly and thoroughly — defensible, efficient, and consistent.

Why

Adaptive

Operations that back you up: you are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.

Clear productivity model with upside: you are paid per productivity point with clear expectations (market-specific).

Schedule flexibility: we build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible.

Cutting-edge tech built for clinicians: charting tools reduce documentation burden, and scheduling (launching soon) will streamline routing/coordination and reduce last-minute changes.

Mission-driven work: we are here to expand access to high-quality home health care, and we want clinicians who take pride in raising the standard.

Market context

Michigan PT roles need licensure and patient focus

Michigan allied-health employers often seek physical therapists for outpatient, inpatient, and rehab settings, with demand shaped by patient volume, aging populations, and care access needs. These roles are competitive because employers look for accredited PT graduates, current or pending Michigan licensure, and a clear commitment to quality outcomes. Review the AI-summarized requirements and benefits here to save research time, then confirm your licensure status and tailor your application to patient-care results.
