  • Type: Full TimeIn the role of the Home Health Physical Therapist (PT), you will provide therapy services to patients in their homes, helping them regain mobility, improve functional independence, manage pain, and enhance their overall quality of life. You will work collaboratively with physicians, nurses, therapists, and caregivers to develop and implement individualized treatment plans that support each patient's goals and promote safe recovery. The ideal candidate is a licensed Physical Therapist with strong clinical assessment skills, a patient-centered approach, and the ability to work independently in a home health setting.

  • Responsibilities: Evaluate patients' physical abilities and develop individualized treatment plans. Provide therapeutic exercises, gait training, balance activities, and mobility interventions. Assess home environments and recommend equipment or modifications to improve safety and independence. Educate patients and caregivers on treatment plans, exercises, and injury prevention strategies. Document patient progress and maintain accurate clinical records in compliance with agency and regulatory standards. Collaborate with interdisciplinary team members to coordinate care and achieve optimal patient outcomes. Monitor patient responses to treatment and modify care plans as needed.
  • QualificationsQualifications: Current Oklahoma license as a Physical Therapist Graduate of an accredited Physical Therapy program Home health experience preferred but not required Current CPR certification Valid drivers license, reliable transportation, and automobile insurance Strong communication, organization, and time-management skills Why Join Us? Requirements

Must be a licensed Physical Therapist in Oklahoma and a graduate of an accredited PT program with current CPR certification. A valid driver's license, reliable transportation, and automobile insurance are required.
