About the role

The LPN contributes to the nursing process by performing patient assessments, gathering data, and collaborating with the healthcare team on care plans. Key duties include administering medications, performing point-of-care testing, and documenting patient intake.

Primary Functions

At the direction and under the supervision of a recognized licensed authority, the licensed practical nurse (LPN) contributes to the nursing process by performing tasks within the framework of case finding, health teaching, supportive and restorative care. The LPN gathers patient data and performs follow-up assessments using the nursing process.

Essential Functions

1.

Gather patient data and collaborate with healthcare team on plan of care 2.

Provides care that meets professional standards of practice and regulatory requirements 3.

Safely administers medications and vaccines as prescribed by the provider 4.

Assists provider with procedures within the scope of the LPN license 5.

Performs point of care testing 6.

Provides age and developmentally appropriate care to primary and specialty care patients 7.

Performs and documents patient intake including but not limited to: height, weight, head circumference, blood pressure, vision and hearing screen, reason for visit, medication reconciliation 8.

Participates in departmental and system wide quality improvement activities 9.

Screens patient and family calls, gathers information, escalates and refers appropriately

Required: Graduate of accredited Licensed Practical Nurse program

Active DE LPN licensure required

Current BLS certification required from American Heart Association.

Requirements

Candidates must be a graduate of an accredited LPN program with an active Delaware LPN license. Current BLS certification from the American Heart Association is also required.
