SUMMARY

The Community Health Worker (CHW) is a frontline public health worker who serves as a trusted liaison between patients, families, UMMA Health, and community-based services. Drawing upon lived experience, cultural understanding, language skills, or close knowledge of the communities UMMA Health serves, the CHW helps patients overcome barriers to care, navigate health and social-service systems, and build the knowledge and confidence needed to achieve their health goals.

Working as a nonclinical member of the interdisciplinary care team, the CHW provides culturally and linguistically responsive outreach, health education, social support, service navigation, advocacy, and follow-up in clinics, community, home, and other approved settings.

Essential duties

Conduct outreach and build trusting relationships with patients, families, and community members.

Identify patient strengths, priorities, barriers to care, and health-related social needs using approved screening and assessment tools.

Connect patients with medical, behavioral health, dental, preventive, and social services, including food, housing, transportation, insurance, and other community resources.

Assist patients with scheduling, applications, referrals, appointment preparation, and navigation of health and social-service systems.

Follow up with patients and service providers to help confirm that referrals and needed services were completed.

Provide culturally appropriate health education, coaching, social support, and self-management assistance consistent with approved care plans and organizational protocols.

Advocate for patient needs and help patients communicate their goals and preferences to the care team.

Participate in interdisciplinary care planning, case conferences, community outreach activities, and health-promotion events.

Maintain accurate, timely, and confidential documentation of outreach, services, referrals, outcomes, and time spent in the electronic health record or other approved systems.

Maintain appropriate professional boundaries and promptly refer clinical, behavioral health, safety, or crisis concerns to qualified personnel.

Perform other related duties within the CHW scope of practice.

Qualifications/Position requirements

To perform effectively in this position, the incumbent must have

Minimum Qualifications

  • High school diploma or equivalent.
  • Lived experience, cultural alignment, language capacity, or a demonstrated close understanding of the population and communities served.
  • Completion of a qualifying CHW certificate program or eligibility under California’s CHW work-experience pathway, including the ability to complete an approved certificate within the required timeframe.
  • Strong communication, relationship-building, advocacy, organization, and problem-solving skills.
  • Ability to work respectfully with people from diverse cultural, linguistic, socioeconomic, and life-experience backgrounds.

  • Ability to use or learn electronic health records, referral systems, and standard office technology.
  • Ability to maintain confidentiality and comply with HIPAA, organizational policies, and documentation requirements.
  • Ability to travel to UMMA Health sites and approved community locations as required.

Preferred Qualifications

  • One year of paid or volunteer experience in community outreach, patient navigation, health education, social services, peer support, or a related role.
  • Familiarity with South Los Angeles communities and local health and social-service resources.
  • Bilingual proficiency in English and Spanish or another language commonly spoken by UMMA Health patients.
  • Experience working in an FQHC, community health center, public health program, or community-based organization.
  • Motivational interviewing, trauma-informed care, or care-navigation training.
