About the role

The therapist provides in-person counseling, diagnostic evaluations, and case management for a diverse population of individuals and families. They collaborate with an interdisciplinary team to develop care plans and maintain accurate clinical documentation.

Job Summary

The LMHC or LCSW will provide in-person professional counseling services, including assessment, counseling, case management, and care coordination to a diverse patient population of individuals and families. The therapist will collaborate with the interdisciplinary team to develop and implement care plans that address the patient’s psychosocial needs, promote well-being, and support overall treatment goals.

Key Responsibilities

Provide initial intake, diagnostic evaluations, treatment plans and progress notes. Complete progress notes within 24 hours of time of service. Maintain client’s charts and keep complete and correct documentation Case management including care coordination, collaborating with referral sources and connecting clients to care. Complete timesheet daily reflecting the billable hours worked

Qualifications

Masters degree in Mental Health Counseling or Social Work Minimum of 2–3 years of experience in mental health services, case management, or program coordination. Experience working in clinical or community mental health settings preferred. Demonstrated ability to coordinate multidisciplinary care and support for individuals with mental health needs. Ability to work with diverse populations and manage sensitive and confidential situations with professionalism and empathy. Knowledge of HIPAA and other regulations related to mental health care. Proficiency with electronic health records (EHR) and standard office software (e.g., Microsoft Office, Google Workspace). Physical Requirements: This position primarily operates in an office environment. The role requires the ability to sit for extended periods, use a computer and other standard office equipment, and occasionally lift or move items up to 10–15 pounds. Walking, bending, and standing may be required. Requirements

Candidates must hold a Master's degree in Mental Health Counseling or Social Work with 2-3 years of experience in mental health services. Proficiency in EHR software and knowledge of HIPAA regulations are required.
