As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday – Friday, dayshift work schedule.
Minimum high school diploma or equivalent required.
One or more years experience in health care education/ marketing / insurance or commensurate educations preferred.
Must be familiar with medical terminology.
Proficient in the use of personal computer.
Requires a high school diploma and a bachelor's degree in social work or a related human services field. Candidates should have one year of supervised healthcare experience and familiarity with medical terminology.
