The RN Case Manager is responsible for coordinating and prescribing nursing care, conducting comprehensive patient assessments, and developing individualized care plans. Key Responsibilities

  • Prescribe and coordinate nursing care in accordance with hospital policies and standards
  • Delegate nursing activities to appropriate team members, ensuring safe and effective patient care
  • Conduct comprehensive assessments of patients to determine their needs
  • Develop, implement, and evaluate individualized care plans
  • Collaborate with interdisciplinary teams to ensure optimal patient outcomes
  • Maintain full access to patient health information, ensuring confidentiality and compliance with privacy regulations
  • Document all nursing activities accurately and timely

Skills and Qualifications

  • Valid and current Registered Nurse license
  • Proven experience in a clinical nursing setting
  • Strong assessment, critical thinking, and clinical decision-making skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively within a team and independently
  • Knowledge of healthcare regulations, standards, and best practices
  • Proficiency in electronic health record (EHR) systems

This position offers a Monday through Friday schedule from 8

  • 00
  • AM to 5:00 PM, providing a balanced work environment and some rotation of weekends. We value our staff and are dedicated to supporting your professional development and well-being as part of our mission to deliver exceptional patient care.

Requirements

Current Registered Nurse Licensure in North Carolina. Certification in BCLS. Successful completion of a hospital and unit-specific orientation which includes age and population specific skills for the area of practice in which the RN will be working and addresses the growth and development needs for the patients for whom the RN will be providing nursing care. Must possess full range of body motion to pass basic FIT test for position to include walking, kneeling, standing, pushing, pulling, bending, stooping, reaching and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds.
