About the Role

As a CNA, you'll play a vital role in supporting our patients' quality of life through consistent, one-on-one, relationship-based care in their homes. Compensation

$16.00-$22.00/hr depending on experience

$1/hr shift differential for nights and weekends

Key Responsibilities

• Assist with activities of daily living (ADLs) (bathing, dressing, grooming, toileting,

  • mobility)
  • Provide companionship, emotional support, and social engagement
  • Support meal preparation, nutrition support and light housekeeping
  • Monitor and report changes in condition
  • Follow care plans and document accurately using EMR standards

Qualifications

  • High school diploma or GED
  • Active CNA certification (CO)
  • CPR certified
  • Valid driver's license and reliable transportation
  • Ability to work independently in a non-facility setting
  • Home health, long-term care, or hospice experience preferred.
