About the role

Provide safe, age-specific nursing care to patients and families to maintain optimal health status. Deliver relationship-based care and maintain secure Protected Health Information (PHI) while collaborating with clinical operations.

Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.

Functions in accordance with the professional standards of the Ohio board of nursing.

Completes/submits required documentation.

Delivers effective relationship based nursing care.

Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.

Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations.

Develops self and contributes to the professional practice of nursing.

Displays leadership that influences optimal clinical and operational outcomes.

Performs other duties as assigned.

Complies with all policies and standards.

For specific duties and responsibilities, refer to documentation provided by the department during orientation.

Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.

Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Full time 40 hours per week.

Monday-Friday with rotationg weekends and holidays.

Covering Geauga, Portage and Summit Counties and surrounding areas.

100%25 travel to the patients homes for care.

30 visits per week.

Adult population.

Other Accredited Program

  • Completion of accredited practical nurse program or currently enrolled in a nursing program required. 2+ years Nursing Experience (Level II only). Knowledge of LPN scope of practice according to the Ohio board of nursing. Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).

Requirements

Requires a Licensed Practical Nurse (LPN) license in Ohio or a Multi-State Compact license and Basic Life Support (BLS) certification. Candidates must have completed an accredited practical nurse program and 2+ years of experience for Level II positions.
