The RN will coordinate care for patients with chronic and complex rheumatology conditions through clinical triage, assessment, and education. Responsibilities include managing patient portal communications, conducting nurse visits, and collaborating with a multidisciplinary care team.
Current Massachusetts Registered Nurse (RN) license. Current AHA BLS certification. Minimum of two (2) years of RN experience. Strong assessment, triage, patient education, and care coordination skills. Excellent verbal, written, and interpersonal communication skills. Demonstrated ability to collaborate effectively with providers and interdisciplinary teams. Commitment to delivering compassionate, patient-centered care to individuals from diverse backgrounds. Ambulatory or specialty practice nursing experience. Experience caring for patients with chronic or complex medical conditions.
