High Point Residence Peoria is seeking a compassionate, dependable, and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our clinical team. This full-time position is an essential part of providing exceptional, person-centered care within our supportive living community.
Working under the direction of the Wellness Director, you will provide quality nursing care, oversee resident wellness, administer medications, and collaborate with caregivers and interdisciplinary team members to promote the highest level of health, comfort, and dignity for every resident.

Primary responsibilities
Provide direct nursing care in accordance with individualized resident care plans.
Perform resident assessments, monitor vital signs, and identify changes in condition.
Administer medications and treatments accurately while maintaining timely and complete documentation.
Communicate resident status and concerns with physicians, families, and the interdisciplinary care team.
Supervise, support, and provide clinical guidance to Certified Nursing Assistants and direct care staff.
Promote resident safety while ensuring compliance with all nursing standards and community policies.
Assist with admissions, resident evaluations, and care planning as needed.
Complete additional duties assigned by the Executive Director or Wellness Director.
