Center for Human Development (CHD) is seeking a full time Licensed Practical Nurse (LPN) for our Congregate Care Network (CCN). Join a collaborative residential services team supporting youth in DCF congregate care programs. The LPN provides hands-on quality assurance for medication administration systems across regional program sites. This role combines clinical oversight, staff coaching, record review and coordination with prescribers, pharmacies and community providers to help ensure safe, compliant and compassionate care.
What You’ll Do
Review medication administration practices at assigned residential sites for compliance with prescriber orders, DCF requirements, licensing standards, MAP policies and CHD’s Medication Administration Program.
Verify that medications are ordered, received, stored, documented and administered correctly, and that pharmacy-dispensed medications match current prescriber orders.
Monitor clinical protocols related to vital signs, blood glucose, allergies, side effects, liquid medications and required prescriber notification.
Observe certified staff administering medication and complete required annual competency observations and documentation.
Identify training needs and provide coaching, in-service education and technical assistance in partnership with program supervisors and the RN.
Review and follow up on Medication Occurrence Reports (MORs), identify patterns and recommend training or other corrective intervention.
Support medication reorders, medication reviews, prescriber paperwork and appointment coordination with primary care and other outside providers.
Maintain oversight of medication rooms; verify that required policies, manuals, certifications and program registrations are current; and communicate deficiencies or technical-assistance findings to program leaders and state MAP contacts in accordance with policy.
