Job Requirements A Day in the Life of a CRMG Patient Navigator

The CRMG Patient Navigator serves as a Medical Assistant with expanded responsibilities supporting specialty clinic program(s). This role provides clinical support, patient navigation, care coordination, and program tracking for patients throughout the program's continuum of care. In addition to standard Medical Assistant duties, this position plays a critical role in multidisciplinary coordination, quality improvement, patient education reinforcement, and proactive follow up to ensure patients progress appropriately through program requirements and treatment plans.

Maintains and updates patient checklists to ensure required steps (visits, testing, education, approvals) are completed accurately and timely.

Performs detailed chart review and chart preparation prior to patient appointments to support efficient provider visits in addition to regular chart audits and reviews patient progress.

Identifies patients who are not progressing as expected and proactively follows up clarify, reinforce, address barriers, and coordinate next steps.

Serves as a consistent point of contact for patients to support understanding, engagement, and continuity of care and supports in patient and family education by reinforcing provider directed teaching and program expectations.

Obtains prior authorizations for medications and related services as required by patient insurance plans.

Coordinates referrals to internal and external providers and services as directed by providers and program standards.

Participates in multidisciplinary team meetings to improve patient care processes, outcomes, and program efficiency.

Partners with the Medical Director and clinic and program leaders to implement and maintain quality improvement initiatives in alignment with CRMC and program standards.

Assists with monitoring compliance with accreditation, regulatory, and organizational standards as applicable to the program(s).

Serves as a liaison between patients and the multidisciplinary care team Desired Skills: Knowledge of medical record documentation requirements and federal/state regulations governing protected health information Strong knowledge and understanding of bariatric medical and surgical pathways Ability to analyze options and coordinate referrals to appropriate providers and resources Ability to engage patients and families in understanding and following a plan of care Strong care coordination, problem solving, and time management skills Ability to read and interpret provider orders, operative notes, and interdisciplinary documentation Excellent organizational skills with attention to detail Ability to manage multiple priorities in a fast paced specialty clinic environment Knowledge of bariatric and surgical workflows Here is What You Need: High school diploma (or equivalent certificate from an accredited program) or higher degree Current Medical Assistant (MA) certification One (1) or more years of experience in a medical specialty clinic One (1) or more years of experience with clinical program oversight, quality improvement, or care coordination Weight Loss Clinic: One (1) or more years of experience in bariatric, weight management, or surgical programs About Cheyenne Regional: Cheyenne Regional Medical Center was founded in 1867 as a tent hospital by the Union Pacific Railroad to treat workers injured while building the transcontinental railroad.

Requirements

Candidates must have a high school diploma and a current Medical Assistant certification. At least one year of experience in a medical specialty clinic and experience with clinical program oversight or bariatric programs is required.
