Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.
A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP) 1+ years of experience in inpatient rehab Acute Care Experience Must have the ability to pass a background check.
