The Shelter Case Manager plays a key role in supporting residents by assessing needs, developing individualized service plans, and connecting clients to housing, medical, mental health, and employment resources. This position focuses on empowering individuals toward stability and self-sufficiency while maintaining accurate documentation, ensuring compliance with program requirements, and collaborating with a multidisciplinary team to provide comprehensive, client-centered care.
Reporting relationship: Shelter Case Manager reports to the Director of Social Services
10AM - 6PM
11AM - 7PM
12PM - 8PM
1-2 Late days per week , one weekend day (selected sites)
Par Central - Jamaica
Fulltime Positions, one weekend day and 1-2 late days per week, selected sites may require a weekend day.
Develop and monitor individualized service and treatment plans, including regular resident check-ins, assessments, and crisis intervention as needed
Connect residents to community resources, social supports, and essential services that promote independence and well-being
Coordinate and track services, including follow-ups with referrals and ongoing communication with providers
Maintain accurate documentation, resident records, and provide timely reports to supervisors
