This flexible position requires availability 2 days a week between 8:00 AM – 8:00 PM, with travel within a 30-mile radius. Ideally, we're seeking someone who would case manage 4-5 clients. Responsibilities: Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards. Active RN license in the state of Minnesota. Previous experience in a home health or acute care setting is advantageous. Exceptional interpersonal and communication skills. Committed to delivering high-quality, patient-centered care.
