The Clinical Pharmacist provides individualized health promotion and disease prevention across multiple sites, including administering vaccines and HIV treatments. They collaborate with providers to implement best practices and ensure consistent application of organizational policies.
A Clinical Pharmacist – Multisite contributes to AHF by expanding primary care functions of pharmacists at more than one site to provide individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF providers.
A Clinical Pharmacist – Multisite enhances the reputation of AHF by providing valuable contributions to more than one location by bringing “best practice” and collaborative ideas, as well as providing consistent application of processes, procedures and policy throughout the organization.
