What You’ll Be Responsible For
Serve as a contributing member to the PACE Interdisciplinary Team (IDT).
Provide direct, hands-on clinical care to residents, including assistance with activities of daily living such as bathing, dressing, grooming, and mobility
Support residents’ physical and emotional well-being by building meaningful connections and delivering compassionate, person-centered care
Monitor and report changes in residents’ condition to the nursing team to ensure timely and appropriate care
Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards

As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
Located in the welcoming community of Florence, PACE is part of the greater Northern Kentucky region.

Candidates must hold a Master's Degree in Social Work and a current LCSW license in Kentucky. A valid driver's license, background check clearance, and up-to-date immunizations are also required.
