About the role

Provide professional overnight nursing care, including assessments and medication administration, for hospice patients and their families. Coordinate with physicians and community partners to ensure continuity of care and accurate clinical documentation.

Description Coverage Territory

Bloomington, Pontiac, Streator, Seneca, Ottawa, LaSalle Peru, Dwight, Marseilles, and surrounding areas.

Schedule: 8 PM–8 AM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion.

Your responsibilities include: Delivering professional nursing care across the full nursing process within your scope of practice Conducting thorough patient and family assessments and contributing to individualized care plans Completing all visit types as needed to meet overnight patient and family needs Administering medications and treatments per physician orders Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment Recording accurate, timely clinical notes including pain and symptom management outcomes Communicating changes in patient condition to physicians and obtaining orders as required Coordinating with community health partners to support continuity of care Ensuring equipment and necessary services are in place for patients Maintaining and growing your clinical skills through continuing education and in-service participation Benefits We Offer Competitive pay Medical / Dental / Vision Insurance plans Long & Short-Term Disability 401(k) with company match Auto Allowance & Gas Card Company provided equipment Paid Time Off Why Choose Transitions Hospice?

Requirements

What You Bring Active LPN license in the state of employment Graduate of an accredited school of Nursing. Clinical experience in hospice, home health, or a related setting preferred Comfort and composure working independently during overnight hours Strong assessment skills and sound clinical judgment in high-pressure moments A calm, reassuring presence for patients and families during nighttime distress Excellent documentation habits and communication skills Valid driver's license and willingness to travel locally
