The Hospice RN Case Manager provides skilled nursing care and symptom management for patients in various home and facility settings. They coordinate care with an interdisciplinary team and serve as the primary clinical coordinator for patients and their families.

We're seeking a Registered Nurse (RN) Hospice Case Manager to join our growing hospice team and make a lasting difference in the lives of patients and their loved ones.

In this rewarding field-based role, you'll manage a dedicated caseload of hospice patients throughout Southern Fairfield County, including Norwalk, Westport, Darien, Stamford, Greenwich, and surrounding communities.

Working as part of an experienced interdisciplinary team, you'll coordinate care, advocate for your patients, and provide expert clinical guidance during one of life's most important journeys.

What You'll Do As a Hospice RN Case Manager, you'll provide skilled nursing care while serving as the primary clinical coordinator for your patients and families.

Patient Care & Case Management Conduct hospice admissions, comprehensive assessments, and ongoing patient evaluations.

Develop, implement, and update individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary hospice team.

Provide skilled nursing care and expert symptom management in patients' homes, assisted living communities, skilled nursing facilities, and other care settings.

Monitor changes in patient condition, communicate with physicians and team members, and adjust care plans to meet evolving needs.

Educate and support patients and families regarding disease progression, medications, comfort measures, and end-of-life care.

Provide compassionate emotional support and guidance to patients and loved ones throughout the hospice journey.

Care Coordination & Clinical Excellence Coordinate care across nursing, social work, hospice aides, chaplaincy, volunteers, and therapy services to ensure seamless, patient-centered care.

Participate in interdisciplinary team (IDG) meetings and contribute to collaborative care planning.

Supervise hospice aides and ensure care is delivered in accordance with the established plan of care.

Maintain timely, accurate documentation that meets Medicare, CHAP, and agency standards.

Ensure compliance with all clinical, regulatory, infection prevention, and HIPAA requirements.

Team & Community Collaboration Serve as a trusted clinical resource for patients, caregivers, and fellow team members.

Participate in an on-call rotation to support patients during evenings, weekends, and holidays as scheduled.

Assist with orientation, mentoring, and education of new staff when appropriate.

Represent RVNAhealth professionally within the community and among healthcare referral partners.

What You'll Bring Graduate of an accredited School of Nursing (BSN preferred).

Current Connecticut RN license or Compact RN License.

Minimum of 2 years of nursing experience.

Hospice, palliative care, home health, oncology, ICU, or other complex clinical experience strongly preferred.

Excellent assessment, critical thinking, and communication skills.

Ability to work independently while collaborating closely with an interdisciplinary team.

Experience with electronic medical records and mobile documentation.

Current CPR certification.

Valid driver's license, reliable transportation, and willingness to travel throughout Southern Fairfield County.

Successful completion of RVNAhealth's hospice orientation and training program upon hire.

Compensation

The pay range for this position is $103,500 to $111,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications. Requirements

Candidates must be a Registered Nurse with a current Connecticut or Compact license and at least 2 years of nursing experience. A BSN and experience in hospice, palliative care, or ICU are strongly preferred, along with a valid driver's license.
