FUNCTIONS: Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual. Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated. Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated. Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment. Educate staff, family, and care givers as indicated. Identify needs for program development and implement programs according to company policies and procedures. Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy. Complete all required documentation according to guidelines specified in Documentation Manual. Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents. Attend all facility required meetings. Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month. Attend Continuing Education courses specific to current position. Maintain professionalism and follow all national/state associations regulations at all times. Supervise student affiliates as directed. All other duties as assigned or directed.
QualificationsQUALIFICATIONS: Must have a degree in Physical Therapy from an accredited program. Active state license required. Excellent organizational and time management skills. Excellent written and verbal communication skills. Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines. Ability to supervise PTAs and PT Techs PHYSICAL DEMANDS AND REQUIREMENTS Ability to stand, walk, sit, stoop, bend, and reach for extended periods. Ability to lift up to 35 pounds frequently (up to 66% of the time spent in performance of duties) and up to 50 pounds occasionally (up to 33% of time spent in performance of duties). Manual dexterity that allows grasping, carrying, and holding equipment and supplies as needed to perform tasks. Ability to see, hear, and verbally communicate with residents, visitors, and staff members during performance of duties.
