The Registered Nurse serves as a Health Services Case Manager providing direct hospice nursing care to patients and their families. They are responsible for planning and directing hospice care and supervising Hospice Aides in the home setting.

The Registered Nurse (RN) Health Services Case Manager provides direct hospice nursing care to patients and families/caregivers.
Plans, organizes and directs hospice care, and Hospice Aide care in the home.
Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations governing Hospice and as may be required by the Hospice Clinical Director to ensure the highest degree of quality care is maintained.
Is experienced in nursing with emphasis on community health and hospice education/experience.
The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families/caregivers within their homes.
