We're seeking experienced and compassionate Registered Nurse (RN) Hospice Case Managers to join our growing hospice team.

Opportunities are available throughout Fairfield and Litchfield Counties, and we'll work with you to assign a territory that best aligns with your location whenever possible.

Our service area includes communities such as New Milford, Sherman, Newtown, Bethel, Brookfield, Ridgefield, Redding, Weston, Wilton, Westport, Norwalk, New Canaan, Darien, Stamford, Greenwich, and surrounding towns.

What You'll Do As a Hospice RN Case Manager, you'll serve as the primary clinical coordinator for your patient caseload, ensuring compassionate, individualized care while working closely with an interdisciplinary hospice team.

Patient Care & Case Management Conduct hospice admissions, comprehensive assessments, and ongoing patient evaluations.

Develop, implement, and revise individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary hospice team.

Provide skilled nursing care and expert symptom management in patients' homes, assisted living communities, skilled nursing facilities, hospitals, and other care settings.

Monitor changes in condition, communicate with physicians and team members, and respond proactively to evolving patient needs.

Educate patients and caregivers regarding disease progression, medications, symptom management, comfort measures, and end-of-life care.

Provide emotional support, guidance, and compassionate counseling to patients and families throughout the hospice journey.

Care Coordination & Clinical Excellence Coordinate care among nurses, social workers, hospice aides, chaplains, volunteers, physicians, and other members of the interdisciplinary team.

Participate in Interdisciplinary Group (IDG) meetings and collaborative care planning.

Supervise hospice aides and ensure care is delivered in accordance with each patient's plan of care.

Maintain accurate, timely clinical documentation that meets Medicare, CHAP, and agency standards.

Ensure compliance with all regulatory, infection prevention, HIPAA, and agency policies.

Team Collaboration Serve as a trusted clinical resource for patients, caregivers, and colleagues.

Participate in an on-call rotation to provide support during evenings, weekends, and holidays as scheduled.

Mentor and assist with orientation of new staff as appropriate.

Represent RVNAhealth with professionalism and compassion throughout the communities we serve.

What You'll Bring Graduate of an accredited School of Nursing (BSN preferred).

Current Connecticut RN license or Compact RN License.

Minimum of two years of nursing experience.

Hospice, palliative care, home health, oncology, ICU, medical-surgical, or other complex clinical experience preferred.

Excellent clinical assessment, critical thinking, and communication skills.

Ability to work independently while collaborating closely with an interdisciplinary team.

Experience with electronic medical records and mobile documentation.

Current CPR certification.

Valid driver's license, reliable transportation, and willingness to travel within an assigned territory.

The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications. Requirements

Requires a nursing degree and a current Connecticut or Compact RN license with at least two years of nursing experience. Must possess a valid driver's license and be willing to travel within assigned territories in Fairfield and Litchfield Counties.
