About the role

The Admissions RN assesses patient eligibility for hospice care and guides families through the admissions process. They collaborate with physicians and interdisciplinary teams to ensure a smooth transition and high-quality care handoff.

Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most. What You'll Do as a Hospice RN Admissions Nurse:
  • Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
  • Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
  • Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
  • Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
  • Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
  • Use the nursing process to assess patient needs, support care planning, and document clinical findings
  • Build trust and provide emotional reassurance to patients and families as they make this important decision
  • Accurately complete required documentation and ensure compliance with state and federal regulations
  • Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff About You

Qualifications

What You’ll Bring:

  • Active RN license in the state of employment (or eligible to obtain)
  • 1+ year of general nursing experience
  • A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
  • Strong assessment, communication, and interpersonal skills
  • Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
  • Valid driver’s license, auto insurance, and reliable transportation
  • Current CPR certification Preferred Experience (Not Required):
  • 2+ years of hospice, home health, healthcare admissions, or medical sales
  • Bachelor’s Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
  • Background in case management, care coordination, or healthcare outreach

Requirements

Candidates must have an active RN license, at least one year of general nursing experience, and a valid driver's license with reliable transportation. CPR certification is required, while a bachelor's degree or CHPN certification is preferred.
