The Patient Navigator assists patients and families in accessing hospital and community services to meet their health and wellness needs. They provide personalized care, conduct health screenings, and cultivate relationships with healthcare providers and community partners.

A Bachelor's Degree in Social Work and BLS certification from the American Heart Association are required. One year of experience in a healthcare setting is preferred.
