Position Overview RN Case Manager Signature Healthcare at Home, part of The Pennant Group, is seeking a full‑time, non‑exempt Registered Nurse Case Manager to support our west‑side Portland service area.

Provide skilled nursing interventions in patient homes, including wound care, medication management, chronic‑condition support, and patient education.
