About the role

The role involves assessing, intervening, and following up with referred patients while providing comprehensive case management and advocacy. The social worker also serves as a liaison between patients and community service systems to develop resource networks.

Assists with assessment, intervention, and follow-up of referred patients. Provides case management including continuum care planning and advocacy. Serves as department community resource development specialist and as a liaison between select patients of all ages and community service systems. Performs other job duties as required.

Qualifications

  • EDUCATION Bachelor of Social Work degree from a program accredited by the Council on Social Work Education or equivalent bachelor's degree with experience in working in a related helping profession or with community agencies.
  • EXPERIENCE May be a new graduate.
  • Preference is given if experienced in health or mental health social work or related helping profession.
  • SPECIAL SKILLS Competency in caring for patients treated at the Hospital regardless of age, disability, sexual preference, gender, ethnicity, or diagnosis.
  • Computer literacy.

Requirements

A Bachelor of Social Work degree from an accredited program or an equivalent degree with related experience is required. New graduates may apply, though preference is given to those with experience in health or mental health social work.
