The Nurse Practitioner performs face-to-face hospice recertification visits and monitors client conditions in designated territories. They are responsible for executing doctor's orders and performing necessary client assessments as part of a multi-disciplinary team.
Jefferson Health at Home by BAYADA Hospice is hiring a compassionate and dedicated per-diem Nurse Practitioner, (NP) to join our team.
This office services our hospice clients throughout Bucks County, Montgomery County, and Northeast Philadelphia.
Prior home care, hospice, or palliative care experience strongly preferred, but not required.
As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Nurse Practitioner (NP) Responsibilities: Making home face to face hospice recertification visits to clients in designated geographic territories.
Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
Qualifications for a Nurse Practitioner (NP): A current license as a NP in PA A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.
Strong interpersonal skills.
