About the role

The Physical Therapist is responsible for conducting resident evaluations and implementing therapy interventions. They develop and revise care plans, establish functional goals, and educate families and healthcare teams on resident care.

Overview The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions.

Responsibilities

Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plans. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.

Qualifications

Qualifications Required Current state licensure in Physical Therapy, in good standing. Bachelor’s degree. EEO/AA/Vet Friendly

Salary Range

Additional Information

Requirements

Candidates must possess a Bachelor's degree and a current state license in Physical Therapy in good standing. The role requires the ability to manage both short-term and long-term resident needs.
