Purpose Under the purview of the Permanent Supportive Housing Department, the Permanent Supportive Housing Case Manager provides case management services to adults experiencing homelessness who will be living in long-term supportive housing arrangements.
The case manager may assist with finding, securing, and maintaining housing while also providing linkage to medical, mental health, and substance abuse resources, entitlement/state benefits assistance, and employment opportunities.
