The LPN provides and documents nursing care for sick and injured patients within their homes to promote health and rehabilitation. Key duties include monitoring vital signs, administering medications, and collaborating with other healthcare providers to coordinate care.

Minimum Requirements: Must have One year acute nursing care, hospital based preferred. Must possess and demonstrate critical thinking skills. Must maintain emotional stability in emergency settings. Experience in home health is required Certified Home Health Required Including Proficiency with OASIS in highly preferred Must have Massachusetts LPN license Infusion Therapy Experience is preferred Demonstrated knowledge of the principles and practices of nursing. Knowledge of and ability to apply community health principles and practice. Demonstrated ability to perform independently as a clinician with minimal supervision or assistance. Experience with self-scheduling of caseload preferred. History of flexibility in assuming varied assignments. Current Driver License (and insurance) [Required] In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines. Traveler candidate must have their own vehicle. This is a firm requirement. Traveler perm address must be included in profile Ability to exercise independent judgement. Demonstrated interpersonal skills. Ability to maintain confidential information Ability to develop and maintain records and communicate clinical data in verbal and written form Willing to be flexible regarding work hours. Ability to drive an automobile in inclement weather. Must be willing to accept “on call” responsibilities. Conforms with Customer Service and Performance Standards. Conforms with Patient’s Rights Policy. History of dependability in meeting demands of committed work schedule. Must have and maintain current Basic Life Support (BLS) certification during employment Excellent communication skills. Ability to work independently. Strong organizational skills and ability to multitask Willingness to participate in continuing education and training programs Strong attention to detail and ability to work independently Strong communication, professionalism, and reliability Compliance/Screening (Pre-Start): Drug screen and background check (facility required) Additional facility onboarding forms as assigned
