Conduct home visits with patients throughout the Niagara area in New York City. Build trusting relationships with patients and their families. Must be fluent in both English and Spanish. Valid New York State driver's license with reliable transportation. Willing and able to travel throughout Yorktown Heights and surrounding areas High school diploma or GED required. Previous experience as a Community Health Worker, Case Manager, Patient Navigator, Social Services Worker, or in a related healthcare field preferred. Excellent communication and interpersonal skills. Ability to work independently and manage a flexible daily schedule. Basic computer and documentation skills.
