Conduct health assessments, physical examinations, Good Faith Exams, laboratory reviews, and follow-up appointments Evaluate symptoms, health history, medications, risks, lifestyle, goals, and diagnostic findings Interpret comprehensive bloodwork and advanced urine testing Identify meaningful patterns beyond isolated laboratory reference-range flags Develop individualized treatment plans using appropriate combinations of hormone optimization, peptide therapy, medical weight loss, lifestyle support, supplementation, IV therapy, and other available modalities Prescribe and manage hormone optimization plans for eligible women and men Evaluate and support women experiencing perimenopause, menopause, and other hormonal changes Prescribe, monitor, and adjust medical weight loss and peptide therapies when clinically appropriate Educate clients regarding findings, treatment options, risks, benefits, expectations, and follow-up requirements Monitor response through symptoms, progress, repeat testing, adherence, and side-effect assessment Adjust treatment based on the client’s response and ongoing clinical evaluation Recognize findings requiring further evaluation, referral, or escalation Remain engaged with current clinical research and continuing education Attendance and participation in training or educational opportunities with Relive Health Responsibly apply newly gained knowledge to appropriate client cases Collaborate with the physician, clinical team, pharmacy partners, and location staff Complete accurate same-day documentation Maintain HIPAA, infection-control, safety, and compliance standards
Graduate of an accredited Nurse Practitioner or Physician Assistant program with active, unrestricted Pennsylvania licensure and current national certification Current prescribing authority and any required collaborative, written, or supervisory agreements Ability to obtain and maintain DEA registration if requested Three or more years of relevant clinical experience preferred Experience in functional or integrative medicine, hormone optimization, women’s health, medical weight loss, metabolic health, or a related field preferred Experience managing hormone therapy for women and men, interpreting comprehensive diagnostic testing, and prescribing peptide therapies strongly preferred Strong clinical judgment related to client selection, contraindications, informed consent, monitoring, documentation, and follow-up Commitment to continued learning and responsible application of Relive’s education, clinical resources, pharmacy relationships, and collaborative support Strong communication, teamwork, and technology skills, including comfort with EHR, laboratory, pharmacy, and prescribing platforms Ability to maintain all required licensure, certifications, training, and continuing education and successfully complete a background check Aesthetic injectable experience is a plus
