We’re looking for a compassionate and motivated Registered Nurse Case Manager to join our team, supporting Northern Virginia.
This is a field-based role that blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.
Experience in case management, care coordination and/or managed care organizations. Discharge Planning and/or transition of care experience Bilingual (English/Spanish) Ability to thrive in a fast-paced, dynamic environment and manage multiple priorities effectively Education: Associate Degree in Nursing (required) Bachelor of Science in Nursing (BSN) (preferred) Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
