The RN will perform independent nurse visits for wound care and chronic disease management while administering medications and injections. They will also triage walk-in patients and collaborate with providers to evaluate and treat acute patient needs.

You will attend to the needs of the team including injections, medication administration and patient education.
You will get to work at the top of your scope doing independent Nurse Visits such as wound care, managing INRs, Hypertension visits, UTI’s, etc.
You will follow up with abnormal lab results and treating per protocol.
You get to do Co-visits where you will evaluate patients, in collaboration with the provider, and treat patients with acute needs to help create access for patients.
You will triage any walk-in patients.
You will build teamwork by attending huddles before the start of shifts to review any anticipated nurse needs during that shift.
Approximately $34.70 - $43.95 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
Bilingual in English and Spanish required.
